Access to your health records
You have the right to see any of your written health records. To
obtain a copy of your records you will need to submit an
application form from the Legal Services Department.
You can download an electronic copy of the application form
below. Please print off the application form and complete it in
full using black ink and ensure that you include a
signature before submitting it to the Legal Services
Please be aware that the application form must be signed by hand
using black ink before it can be submitted. You may wish to
complete parts of the application form electroncially however it
must be signed using ink and submitted as a hard copy document in
paper form. Any submissions via email will not be accepted.
