Tarporley War Memorial Hospital serves as the rural base for our Neighbourhood Community Services, connecting healthcare directly to local residents in their own homes. Community Services based at Tarporley War Memorial Hospital include:
Community Response Hub:
Our Rural Community Response Hub provides care to patients in their own home to prevent an unnecessary hospital admission. They also support local residents to discharge from hospital and provide care at home to enable recovery in their own environment. The team also provides Palliative Care in Partnership (PCIP) for people accessing Continuing Health Care (CHC) and have chosen home as their preferred place of care.
Care Community Team:
Care Community Teams are a group of health and social care professionals who work alongside your local GP Practice to support patients at home. The team includes nurses, therapists and social workers. They also have strong links with many other health, local authority and voluntary agency teams.
In addition to our community teams, a number of outpatient clinics are held at Tarporley Hospital where patients would otherwise be seen at our main hospital site.
